Dr. Concepcion has a particular interest in preventive medicine and places tremendous value on nutrition and holistic medicine. Because of his genuine interest, he is the director of our medical nutrition program.

Dr. Lee is a board certified family practice physician. He enjoys his status as a general primary care provider because he is able to participate in the preventative medical care of patients of all ages. From calming the nerves of new parents and tending to the needs of newborns to being the medical director at a skilled nursing facility. He graduated from UC San Diego and completed his residency at UC Davis Medical Center. Dr. Lee practices and lives by the motto “The meaning of life is to have a life full of meaning.” River Bend strives to provide a much needed environment of genuine care, and Dr. Lee exemplifies this.
Fernanda Da Silva M.D.

Dr. Fernanda Da Silva was born in California to Portuguese immigrant parents. She grew up on a dairy farm in the Central Valley town of Merced. And yes, she grew up milking cows and feeding calves. Her parents valued hard work but they also valued an education. When at age six she told them that she wanted to be a doctor, they encouraged her and supported her.

Dr. Da Silva joined River Bend Medical Associates in 1999. She is board certified in Family Practice. Her areas of interest include hospital medicine, women’s health, and rheumatological disorders. She maintains medical staff privileges at Mercy General and Methodist Hospitals as well as courtesy privileges at Sutter Medical Center. She is devoted to sharing her knowledge of the art of medicine. She attends on the family practice service at Mercy General Hospital and has helped train nurse practitioners and physician assistants.
Dr. Da Silva has been a resident of Sacramento since 1996. In her free time she enjoys all that Sacramento has to offer. Sundays, after church, you can often find her at the farmers market or the antique faire. Recently, you may have also seen her and her family volunteering to dead head roses at the old city cemetery or pulling invasive weeds on the American River Park Way. She and her husband are particularly fond of the activities found in their Curtis Park neighborhood.

Being a bilingual spanish speaking provider, he is able to provide culturally sensitive care to diverse communities. Dr. Lizarraga’s favorite courses of study are injuries and the science behind how families cope with serious illnesses and traumatic injuries.
Dr. Lizarraga is a very proud grandfather. He spends much of his time helping with his grandchild. Traveling and cruising are experiences he loves to share with his family. A favorite destination is Cancun, and snorkeling is among one of his favorite memories of his time in the Yucatán peninsula. Dr. L also enjoys experiencing his local communities, especially the local culinary scene, and he aims to eat at all the restaurants in Sacramento. In addition to being a foodie, he loves classical music and classic rock.
Helping patients find balance and wellness is his top priority, and he looks forward to getting to know the patients in our practice.

She provides information clearly to her patients during their visit then follows up with additional educational resources such as handouts or online content. Dr. Correll also enjoys working with geriatric patients that often require management of complex medical issues.
Dr. Correll was born in Poland and immigrated to the US with her family as a child. She is fluent in English and Polish. Dr. Correll recently relocated to the Sacramento metropolitan area with her husband and daughter. She loves the sense of community and all the great things to do in and around Sacramento. Some of her hobbies are yoga, reading, listening to music, and going on walks with her family and dog.

Kristian Borofka was born and raised in Clovis, California, 3 hours south of Sacramento. She went to medical school at Touro University California after she completed a BS in Biochemistry at Santa Clara University. Her love and interest in medicine began long before her higher education; she was diagnosed with Celiac’s disease (an autoimmune disorder related to a common food product-gluten) at the age of 16. This inspired her life-long interest in the interplay between lifestyle changes and physical and emotional health. While in medical school and residency her passions grew and now encompass LGBTQ+, geriatric, and women’s health issues. Her favorite part of medical practice is establishing relationships with her patients and families, and helping individuals better understand and manage their own health. Outside of medicine, she enjoys spending time with her family and friends, knitting, exercising and swimming, and cooking.
Carmelita Lombera

Carmelita is a graduate of UC Berkley, and started her journey in the Wellness industry as a fitness instructor at the YMCA as she completed her Bachelor’s Degree in Nutrition.
Carmelita takes pride in seeing a need and meeting it. She is most fulfilled by the joy that comes with patient transformations, as well as patients being able to reduce their medications. Human connection is the absolute core of her practice style. She is fluent in Spanish, which helps her to make herself accessible to a larger demographic.
She loves hiking in Placerville or Auburn and taking yoga classes. Creating recipes is an absolute passion for Carmelita as well, as she is very interested in how the body responds to food and lifestyle modifications.
Carmelita loves spicy foods, her cat Lola (thanks Happy Tails), and traveling.
Jayanthi Linert PA-C

J.J. originally started her journey in healthcare on track to attend medical school, but she met a PA while working in a family practice clinic and decided that that was a better fit. She wanted to focus on patient care and education, without the added administrative duties that come with being a medical doctor. Her favorite part of being a PA is the teaching element. J.J. Loves spending time with her family, as well has spending time outdoors running or hiking. Janet is an AANP board certified family nurse practitioner. Originally from Los Angeles, Janet received a Bachelor’s degree in Science from University of California, Los Angeles (UCLA) in 2013 and earned her Masters of Science in Nursing degree from Samuel Merritt University (SMU) in 2018. While at Samuel Merritt University, Janet developed a strong passion for working with a wide range of patients, from newborns to geriatrics, in the Family Medicine department. Janet believes in not only achieving the optimal well-being of her patients, but also building rapport with her patients. She enjoys traveling, exploring new cafes, and trying new foods. Meng-Chieh Yang is a family nurse practitioner. She attended Emory University for her master’s in nursing in Atlanta, Georgia. She chose to be a nurse practitioner specifically because nursing emphasized a more holistic approach towards the health and well-being of the whole person. The most rewarding aspect of her job is building relationships with patients. She enjoys cooking, trying new restaurants/food, exploring nature/hiking, and traveling to new places.

Kara grew up in Texas and then moved to Seattle after college where she lived for 8 years before moving to Sacramento.
She received a bachelor of science in Wildlife Ecology from Texas A&M University and a masters from Samuel Merritt University in 2019. John “Jay “ Hume was born in the US territory of Guam. His father was a Naval Officer and his mother is a retired teacher. He has lived the majority of his life in Brazil, Maryland, and Austin, Texas. Jay completed his Bachelor’s degree in Business Administration from Shippensburg University in Pennsylvania and graduated with his Master’s in Health Science from UC Davis. As a Physician Assistant, the aspect of practice he enjoys most is assisting patients with health care decisions, allowing them to live their “best life”. In addition to working in Family Practice and Urgent Care, he serves in the US Army reserves as a Physician Assistant.
